Services Overview

At Promontory Point Rehabilitation, we provide rehabilitative services and skilled nursing care for people who require a regimen of therapy following a recent hospital stay. Our services are for those who no longer need to be in a hospital but are not quite ready or able to go home. We provide the help and information needed to help people get on the road to recovery as quickly as possible: 

Services Include

• Rehabilitation - Including Physical Therapy, Occupational Therapy & Speech Therapy Services - all are available seven days a week
• Skilled nursing care 24 hours a day, seven days a week
• Nutrition counseling and menu planning by our registered dietician
• Rehabilitation education for you and your family
• Transportation to and from medical appointments and the hospital 

Navigating Your Course Home

Our goal - get you back to living the life you want! We have a team of professionals that will create and then implement a plan to get you back home as quickly as possible.

• A home evaluation will be completed by one of our therapists
• Follow-up therapy services will be provided in your own home if needed
• Your medications will be reviewed and changed based on your recovery program
